A courageous 3-year-old boy from Franschhoek, embarked on a challenging journey after a hypoxic brain injury in January 2024. This is the story of his resilience and the dedicated care he received at SJIPC.

Initial Admission: A Tough Start

In February 2024, Teshwin was admitted to SJIPC with severe neurological impairment and feeding difficulties. His condition was characterized by:

  • Low Level of Consciousness: He struggled with neuro-irritability and lacked purposeful movement in his limbs.
  • Visual Impairment: Little to no response to visual stimuli; unable to follow movement or light.
  • Communication Challenges: Absence of intentional vocalization and inability to follow instructions.
  • Feeding Difficulties: Dependent on a temporary nasal feeding tube until PEG surgery.

A Multi-Disciplinary Approach

Teshwin’s care involved a comprehensive approach from SJIPC’s multi-disciplinary team, including:

  • Dietician
  • Nursing
  • Occupational Therapy
  • Physiotherapy
  • Speech Therapy

Therapy goals were set to focus on:

  • Mobilisation and Positioning
  • Communication
  • Safe Feeding
  • Sensorimotor Stimulation
  • Caregiver Training and Education

Progress and Improvement

Post-PEG surgery in May 2024, Teshwin showed significant improvement:

  • Enhanced Alertness and Engagement: Better head control and environmental interaction.
  • Auditory Response: Laughter and babbling in response to verbal engagement.
  • Oral Intake Tolerance: Small amounts of soft food for pleasure, though he still relies on PEG for nutrition.

Despite ongoing visual impairments, Teshwin has shown progress in engagement with his environment, though intentional communication remains a challenge. Alternative communication methods, like a tactile communication board, were introduced but require further development.

Empowering the Family

Teshwin’s mom received training in:

  • Handling and Positioning
  • Feeding Techniques
  • Stimulation and Play Experiences

This training aims to support Teshwin’s continued development and ensure effective care at home.

Looking Ahead

Teshwin has been referred to community services for ongoing therapy. His mom will continue to receive education and support for handling, feeding, and stimulation.
